REQUEST OUR SERVICES

Legal representation by the Division begins with the completion of two forms and the
payment of the service fee.  The CLINIC
Client Agreement Form authorizes the opening of
a new case, and requires the signature of both an authorized official with the
arch/diocese or religious institute and the individual seeking our services.  Both will be
parties in the case.
